1. One Front View Photo: Bite Down & Smile
2. One Right Side Profile: Bite Down & Smile
3. One Left Side Profile: Bite Down & Smile
4. One Front View Photo With Your Mouth Wide Open Exposing Your Top Teeth Interiors
5. One Front View Photo With Your Mouth Wide Open Exposing Your Bottom Teeth Interiors

Step 2. Tell Us What You’d Like To Change About Your Smile.
 
Step 3: That’s it, no obligations! After Dr. Bukholts assesses your photos you will be contacted by our dental practice to set up a virtual consultation. During your virtual consultation you will be informed about  the best treatment options for you and you can discuss any questions you may have with Dr. Bukholts.
